Skilled Nursing
We can help you get home after your hospital stay

 

Clarke County Hospital provides skilled nursing care for patients who require additional hospital care after receiving treatment or undergo a procedure at another healthcare facility.

A skilled level of care means that your initial symptoms have improved, but you are not quite ready to return home. In our skilled nursing care program, your doctor will not see you every day, but will visit with you at least once a week.

Once we have been contacted that you would like to consider coming to Clarke County Hospital for your continued care, we will request a copy of your records to review and make sure your needs can be met here. This process may take up to 24 hours.

We encourage you to bring comfortable clothing for three to four days  to wear during the day. You are in the recovery part of your healing and we want to get you in a routine, much like you will have at home. There are times  that you may leave your room for your therapy and would be more comfortable in your own clothing. All of your clothing will be marked with your name, and will be washed by CCH staff.

At Clarke County Hospital, we want you, your family and loved ones involved in your care plan. You and your family will be involved in a meeting every week with all the departments  that are involved in your care.  These meetings are scheduled with you and your family and usually occur in your room. We will discuss your progress and answer any questions you or your family may have about your care, or any concerns you may have about discharge. During these meetings, we will also talk about a possible date for discharge.

Your skilled stay is driven by the therapies you require as directed by your doctor. As you progress with your therapy, there will be a periodic re-evaluation to verify that you still require a skilled level of care. When you have progressed to your maximum level of recovery, you will be discharged.

Activities
Daily activities to keep you active and busy are planned every day. You are strongly encouraged to participate in these activities. If you have a favorite hobby or past time, please share this with our activity staff, for example card games, cooking, sewing, reading, movies, board games, playing the piano or crafts.

Meals
Three meals a day are served in the Dining Room. you are strongly encouraged not to eat in your room, but at a normal table with others. Snacks are available at any time of the day. Please let the staff know if you have any favorite snacks. We are happy to make your preferred snack as long as the diet the physician has ordered allows you to have it.

Going Home
Starting on admission, the nursing staff and social services will work with you and your family to plan for your return home, including any additional support you may need.
