2019 Community Health Needs Assessment

Every three years, Clarke County Hospital conducts a community health needs assessment survey with Clarke County residents that summarizes the communities feelings on what are essential health and wellness issues in Clarke County. In an effort to expand the survey's reach to County residents, CCH has partnered with the Clarke County Health Department to conduct this year's survey.

From June 20 - August 31, completed surveys will be received through electronic and print mediums from community members.  The survey responses will be collated into a report that will be available in early October.

The survey conducted in 2015 resulted in the development of walking/biking paths in the city of Osceola, health and wellness classes offered to students and their families at area schools, a bike sharing program and many other programs and activities that benefitted Clarke County. Your participation in this survey matters!
